Title Light affects heart rate's 24-h rhythmicity in intensive care unit patients: an observational study Type Journal Article

Abstract BACKGROUND: Intensive care unit (ICU) patients experience two affronts to normal 24-h rhythms: largely internal events such as medication and external factors such as light, noise and nursing interventions. AIMS AND OBJECTIVES: We investigated the impact of light variance within an ICU on 24-h rhythmicity of three key physiological parameters: heart rate (HR), mean arterial blood pressure (MAP) and body temperature (BT) in this patient population. DESIGN: Patients were assigned to beds either in the 'light' or 'dark' side within a single ICU. An actigraph continuously recorded light intensity for a 24-72-h period. METHODS: Measurements of HR, MAP and BT were recorded every 30 min. RESULTS: HR, MAP and BT did not follow 24-h rhythmicity in all patients. Higher light exposure in the Light Side of the ICU (122.3 versus 50.6 lx) was related to higher HR (89.4 versus 79.8 bpm), which may translate to clinically relevant outcomes in a larger sample. Duration of stay, the one clinical outcome measured in this study, showed no significant variation between the groups (p = 0.147). CONCLUSIONS: ICU patients are exposed to varying light intensities depending on bed positioning relative to natural sunlight, affecting the 24-h rhythm of HR. Larger, well-controlled studies also investigating the effect of relevant light intensity are indicated. RELEVANCE TO CLINICAL PRACTICE: Light is a variable that can be manipulated in the constrained environment of an ICU, thus offering an avenue for relatively unobtrusive interventions.

Abstract In this paper, we present the implementation of a monitoring network for artificial light at night (ALAN), based on Sky Quality Meter devices (SQM) installed in seven locations of the Veneto region. The system is coordinated by the Regional Environmental Protection Agency (ARPA-Veneto) and the Department of Physics and Astronomy of the University of Padova, in collaboration with a local dark-sky association, Venetostellato. A new centralized database containing zenith night sky brightness (NSB) data was implemented to collect data from all SQM stations of the regional territory, not only in real time (since 2017), but in some stations since 2011. We now have a dataset to determine how light pollution is affecting astronomical observatories. A WEB portal was created to offer different downloads from these NSB data. We present the results of some elaborations for the 2018 dataset (statistics, histograms, annual and cumulative plots) for seven monitoring sites. For Ekar and Pennar sites, we also present the NSB monthly trend from 2014 until the time of the study. We purchased a reflex camera with a fish eye lens, appropriately calibrated with the software (SW) Sky Quality Camera, which allowed us to study ALAN using differential photometry. Here, we present our first results obtained by studying the night evolution of light pollution in the urban location of Padova.

Abstract Household electricity access data in Africa are scarce, particularly at the subnational level. We followed a model-based Geostatistics approach to produce maps of electricity access between 2000 and 2013 at a 5 km resolution. We collated data from 69 nationally representative household surveys conducted in Africa and incorporated nighttime lights imagery as well as land use and land cover data to produce maps of electricity access between 2000 and 2013. The information produced here can be an aid for understanding of how electricity access has changed in the region during this 14 year period. The resolution and the continental scale makes it possible to combine these data with other sources in applications in the socio-economic field, both at a local or regional level.

Abstract In May 2018, the Federal Institute for Occupational Safety and Health (BAuA, Germany) hosted ‘Light and Health at Work’, a workshop concerning occupational health issues relating to non-visual effects of light of both indoor and shift workers. The agenda reflected a common interest in translational research linking laboratory findings with occupational and public health outcomes, and resulted in the founding of the European scientific network NoVEL (standing for Non-Visual Effects of Light). This article sets out the network participants’ shared goals to improve the scientific evidence about light’s non-visual effects, circadian disruption and well-being, using light exposure interventions with high quality assessment of light.

The main work conditions that impair exposure profiles that support healthy circadian regulation are daytime indoor work that reduces light exposures and night-shift work that increases light-at-night (LAN).

Abstract Light is the most effective environmental stimulus for shifting the mammalian circadian pacemaker. Numerous studies have been conducted across multiple species to delineate wavelength, intensity, duration, and timing contributions to the response of the circadian pacemaker to light. Recent studies have revealed a surprising sensitivity of the human circadian pacemaker to short pulses of light. Such responses have challenged photon counting-based theories of the temporal dynamics of the mammalian circadian system to both short- and long-duration light stimuli. Here, we collate published light exposure data from multiple species, including gerbil, hamster, mouse, and human, to investigate these temporal dynamics and explore how the circadian system integrates light information at both short- and long-duration time scales to produce phase shifts. Based on our investigation of these data sets, we propose 3 new interpretations: (1) intensity and duration are independent factors of total phase shift magnitude, (2) the possibility of a linear/log temporal function of light duration that is universal for all intensities for durations less than approximately 12 min, and (3) a potential universal minimum light duration of ~0.7 sec that describes a “dead zone” of light stimulus. We show that these properties appear to be consistent across mammalian species. These interpretations, if confirmed by further experiments, have important practical implications in terms of understanding the underlying physiology and for the design of lighting regimens to reset the mammalian circadian pacemaker.
